Crystal Run Health Plan, LLC ("CRHP") was formed as a limited liability company under the laws of the State of New York on or about October 3, 2012 and obtained a Certificate of Authority to operate Commercial HMO, Medicaid and Child Health Plus programs on August 1, 2015. Crystal Run Health Transformation Holdings, LLC is the immediate parent and controlling party of CRHP.

CRHP is not authorized to operate in any state other than New York.

CRHP began offering health plans to its members with an effective date of October 1, 2015. On June 30, 2019, CRHP sent notification to its policyholders indicating that the company would begin to withdraw from all its lines of business in the market it served. As of December 1,2020, it had no members

CRHP’s statutory office was located at 155 Crystal Run Road Middletown, New York 10941

CRHP was placed into liquidation and the Superintendent of Financial Services of the State of New York was appointed as liquidator on January 24, 2025, by order of the Supreme Court of the State of New York.
