Historical Information:

Crystal Run Health Insurance Company, Inc. ("CRHIC") was incorporated in the State of New York on December 2, 2013, and obtained on December 31, 2014, a license pursuant to Article 42 of the Insurance Law to transact the business of accident and health insurance, as specified in the Insurance Law § 1113(a)(3)(i). Crystal Run Health Transformation Holdings, LLC is the immediate parent and controlling party of CRHIC.

CRHIC is not authorized to operate in any state other than New York.

CRHIC began offering health plans to its members with an effective date of June 1, 2015. On June 30, 2019, CRHIC sent notification to its policyholders indicating that the company would begin to withdraw from the commercial group market on December 31, 2019. As of December 1,2020, it had no members.

CRHIC’s statutory office was located at 155 Crystal Run Road Middletown, New York 10941.

CRHIC was placed into liquidation and the Superintendent of Financial Services of the State of New York was appointed as liquidator on January 21, 2025, by order of the Supreme Court of the State of New York.
