Historical Information:

Atlantis Health Plan, Inc. ("Atlantis") was incorporated in the State of New York on April 17, 1995, and, on or about May 17, 2000, obtained a Certificate of Authority from the New York State Department of Health to operate as a health maintenance organization pursuant to article 44 of the New York Public Health Law.  Atlantis commenced business on or about September 1, 2000.

Atlantis offered plans providing Medicare Advantage and Medicare Advantage Part D prescription drug coverage and healthcare services to Medicare beneficiaries through a network of hospitals and other healthcare service providers.  Atlantis also provided non-government sponsored coverage to individuals and companies.

Atlantis was not authorized to conduct business in any other state.  Atlantis’ business office was located at 5600 Mariner Street, Suite 200, Tampa, Florida 33609.

Atlantis was placed into liquidation and the Superintendent of Financial Services of the State of New York was appointed as liquidator on April 19, 2019, by an order of the Supreme Court of the State of New York.
